Tri-Form PB, how does this look ?
I'd drop the endmod from both dwarf and nova and slot them in the AoE attacks in the forms instead.
There's a big problem with slots when you're trying to build both your Human Form and your Nova Form for damage output.
I'd say pick either one or the other for damage output and stick with it, your ability to chain attacks really suffers otherwise. With that build's slotting you will have major, major endurance problems. If you pick Human form, you'll need a three-slotted Stamina and Conserve Power, plus Endmods in at least Incandescent Strike/Solar Flare. If you pick Nova Form, you'll be lighter on Endurance use but still probably need endreds in your attacks.
With lessons learned from my 50 Human/Dwarf bi-former, I've got a level 6 tri-former PB with a build plan that essentially has Nova form doing all the damage, with Human form as a Healer/Buffer and Dwarf as a tanker.

Alright, tweaked the build a bit, keeping some things in mind :
In Human form, I want to keep the ability to do some dmg, so the big hitters attacks remain there, with he slotting modified a bit.
I think trying to slot for dmg in Dwarf form was indeed a not so good idea, so reduced the slotting in there. Thinking of using dwarf to either fill in for a tank if the need needs one (=> taunt bot) or when [censored] hits the fan.
Modified the slotting in nova form as well, since it will be my main dmg source.

Lot better than before certainly, looks workable.
I'd try to get Essence Boost 6-slotted tho, the extra HP really makes a difference to your survivability no matter what form you're in. Buffs from Essence Boost, Conserve Power and Hasten all carry over between forms.
The only other major point would be that Dwarfie isn't a damage form. It has three things going for it: Tanking ability, Mez Protection, and an Extra Heal. Slotting the attacks for damage really does you very little good because it does such low base damage to begin with.
Dwarfie's heal is the same as your Human Form one, but works on percentages from your dwarf form's larger HP Pool. The tanking comes down to survivability (slotted heal, resistance SOs in dwarf form, slotted dwarf flare for PBAoE Knockdown) and Aggro control ('mini-gauntlet' from attacks and Antagonise which is NOT autohit).
Ideally for a "tanking" dwarfie you'll want a 6-slotted heal, a fast recharging Flare (2 recharges, one endred and an accuracy seemed to be the best minimum slotting for me), 3 Resistance SOs in dwarf form and at least one accuracy and a few recharges/taunt durations in antagonise.
Also bear in mind that if you respec you can use earlier slots (like the ones in your sheild) in your two forms.
I'd also say unless you're planning on PvPing or going up against +5 mobs you can safely drop the acc from your nova form attacks, they all have an accuracy bonus and you're to-hits in the form will add further to this.
I'd say keep the end mod in the forms rather than end reds in the powers, I find that in nova form I can keep up continual attacks against any mobs - its only when up against end drainers or GMs/AVs that the fight lasts long enough to drop my end - even then if I stick to the single target attacks my end will increase.
I would say my human form was weak until mid 30s but tend to play nova primarily unless the team needs me to tank - (or if the tank gets more aggro than he can handle). The ability to hit dwarf and taunt when the tank drops or preferably before can certainly help prevent a team wipe.

Altered the Dwarf slotting a bit, had to remove an end red from Nova Blast, but can't find a 6th slot for Essence Boost for the life of me. Could possibly take the endred from Dwarf Flare, but I think this build should be ok, with probably a couple tweaks here and then while levelling

You could always use the 3rd slot from Build Up, it only makes about 3-5 seconds difference in the recharge time and you don't have many Human form attacks that will be making regular use of it. I had it on my Human/Dwarf and never three slotted it until my most recent respec.

Ack, just noticed the Dwarfie Taunt...
Play about with the toon for a while and see how your aggro control is, but I'd advise changing that Recharge SO to an Accuracy seeing that Antagonise isn't Autohit. With one accuracy you'll hit most things in PvE (not high defense mobs like MoGed PPs though - be aware! )
Increasing Duration rather than decreasing Recharge will give you better overlap seeing you're short on slots.

The To Hit bonus in Nova form is not huge - I seem to remember seeing that against even level mobs you don't even hit the 95% cap with no acc in the attacks. It's only against higher level foes and when the -def of the attacks themselves kick in that it evens out. With that in mind, you could just slot one of the AoEs with an Acc and use that to add -def for the first shot - the rest should then hit fairly well.
Endred in things like the AoE powers are several times better at saving end than endmod in the forms. They are nice but if you are looking for slots to place elsewhere, they are a good place to start.

well with two to-hits in the form and no accs I still hit in excess of 90% against +2/3, drops off significantly to about 75% against +4s.
Its good enough that my preferred form for taking out voids solo is nova.
Dwarf otoh does need them - with 1 in most attacks I need to stack debuff before they are reliably hitting.

Nova attacks are all standard accuracy, and Nova form unslotted only adds a 9% ToHit chance (nofuture.org)
With 2 Tohit SOs, that will rise to a 13% ToHit buff, which is considerably less than the 18% ToHit you get from unslotted Build Up... it's a much lower constant +Tohit buff than, say, /devices "Targetting Drone".

Wish I could remember how the formula works for anything other than even level mobs so i could check these figures but hey ho.
As I said, you won't hit 90% of even level mobs let alone higher with the first attack. However I'm guessing the attacks have about -9% defence which is about double the benefit that slotting To Hits gives, so as long as that first attack hits....
Hmm I seem to recall it being stated nova had an inherrent acc bonus - the figure I had seen was about 40% which is definitely on the high side but I guess this may have been accounting for to-hit SOs in the form and pre ED 6 slotted that may have been possible.
In any event with the stacking of -def it should still be possible to remove the Accs from powers further down the attack chain so which ones would depend on the chain used and of course for anything you must hit on the first attack - voids, quantums, shadow mobs you will be using BU anyway.

Nova has a 45% damage buff but the figure I always saw listed for To Hit was 12.5%. No Future seems to think it's 9%.
Alrighty, this is first go at a PB, and I like the idea of a triform, even if it means less slots in powers, I like the ability to shift into whatever role is needed.
Don't mind the levels indicated for powers and enhancements, I'll more than likely slot whatever I feel I need at the time.
