The badge is for 6 level 15, 6 level 20 and one other of either level.
There is an issue with the 45-50s, where the badge bar will show you one short, you create a 45 and a 50 and it fails to award. I think the badge actually requires one more of each than the counter suggests, it will eventually award if you create a couple more.
Edit, also, you create 7 level 20s, the counter will show 6, the one of either level doesn't seem to show, but the badge will award after you create the 6 level 15s. At least this is how it worked on all the badges I checked when I got them all recently.

You also need to create 6 level 15 damage IOs
(level 10 common IO badges are the only ones
requiring single levels of enhancements crafted.
The others require 15/20, 25/30, 35/40, 45/50

I apologize if this has been discussed before, but I recently created 7 Level 20 Damage Enhancements to get the badge, and the progress bar never budged. I did it a few days prior, creating 6 Level 20 Damage Enhancements that were counted, but it didn't happen this time.
I did bug it, though I'm considering petitioning it.
Both times I was in my base; both times I purchased recipes straight from the crafting table; both times I was in SG mode.
Is this a bug or a known issue I somehow missed? Your input would be greatly appreciated.
