I don't have the numina's, miracles, or zephyr's yet. This is my first character that I'm dropping IO bombs on, so any tips would be welcome...but no purples. The three previously mentioned sets will be hard enough to scrape together.
just looking at your initial build i see no +recovery slotted at all.
you may need to re-evaluate how much +dmg and +recharge you really need.

Yeah, if you look at the second build, I picked up a lot more regen and recovery (some set bonuses, but mostly from the 3 uniques, which don't show up in the set bonus list, I think).
Well, I don't have MIDS, so I have to pick through these builds by hand, which means I just skim them usually, and I don't get to see the enhancement percentages. A few things jump out about your slotting:
Why FIVE slots in the toggles? The extra Red Fortune in Focused Fighting is extraneous, IMHO, and the other two toggles stop one slot short of granting you a defense bonus. Either 4-slot with LOTGs *or* six slot with Red Fort.
Why five Thunderstrikes in Focus? You are again stopping short of getting the excellent defense bonus, at the penalty of getting Thunderstrike's poor recharge enhancement value in a key power. Six slot Tstrike for the bonus, or better yet, frankenslot for much better recharge/acc/dmg/endred.
Touch of Death gives nice bonuses -- but the recharge is poor, again. Yank it out of Follow-Up, a power that cries out for recharge enhancement. Put it into...say...Strike, six-slotting it with the slots you'll pull out of your toggles and Tough. You're low on attacks anyway, and have slotted low-recharge sets into many of them; another six-slotted attack will be welcome.
Tough can be a mule for the Steadfast unique, or slotted up for resistance. You are too starved for slots to piddle around with the small set bonus for the rest of the Steadfast Protection set -- pull those slots out or use them for full resistance. Given your slotting shortage, I'd just stick the Steadfast unique into the default. At any rate, you do NOT need the knockback protection IO there, and it'll cost a lot. If you want another 1.5% recovery there are other places to pick it up. (Miracles in place of two of the Regen Tissues in Aid Self, for example). And pull that Regen Tissue unique out of Aid Self and put it into Health, please. Save yourself having to spam the heal to keep the bonus.
Check whether Practiced Brawler recharges optimally -- especially if you plan to exemplar. It might be worth a more traditional two-slotting.

I 5-slotted the Red's Fortunes since that gets me 5% recharge, and I'm at def soft-cap already (unless something hits me with def debuffs fairly regularly). I threw the extra one in focused fighting to save end and squeeze out all the def I could.
I think the Thunderstrike 5-slot has some bonus I wanted, but didn't need the defense bonus. I'm at work atm, and can't pull up Mid's. For the FU-Slash-Focus-Slash chain, I think my global recharge will cover it...it almost does now, and I don't have Hasten (which I will in the respec). I purposefully dropped slots from Strike so I could have 5 slots to spend for set bonuses/uniques in Health elsewhere.
I used ToD instead of something with spiffier recharge (currently I has CI and some procs I think?) due to the fact that I really need the defense bonus from six-slotting.
Re: Tough: I rarely ever activate it. I noticed that having 3 steadfast gave similar bonuses as 1 steadfast and 3 titanium (like 3% less resistance), so I dropped one slot out of it, not thinking of the cost of the -kb. Maybe I should just one slot it...or go back to 4 slotting it.
Good point on the Regen unique. Not sure what I was thinking.
I do plan on having a second build for exemping down once I get this build going. Go for level 30 or something for the IO levels, pick up six slotted strike, ignore the 40+ abilities, etc.

Here's a slightly different version of Stinky Claws (as I dub thee). Dropped Aid Another and Aid Self ... for now. I was coming up about 5% shy of the soft cap while keeping them (and without pretty much rebuilding from the ground up which I wished to avoid). That 5% translates to roughly twice the damage taken leading to sort of a self fulling usage of the Medicine Pool. So bye bye Med pool hello Combat Jumping and ... hmmm Confront?
Anyway fodder for thinking on.

If you can soft cap your Claws/SR without taking the Fighting pool and Weave, it gonna be easier on the endurance side. At the same time it will let you more space for other pools (Medecine).
Maybe I missed something but without slotting in Strike your second build really miss a single target attack chain, which really sucks for a Claws/SR because it's what it is supposed to be excellent at.

I have a non-medicine pool build on my BS/SR. I can't PvP with it. I can't take out AVs solo with it. But for anything short of that, what I noticed is that if I have softcapped defenses and IO slotting for regen, then basically all I need to do it eat a green every now and then. A build without sofcapped defenses is going to need to pop a purple on rare occasion. A build with no recovery slotting will have to pop a blue every now and then. None of this is really a problem. People get too dependent on the self heal.
I really like the way my BS/SR runs without the self-heal. And I have aid self for my anti-AV build.
Same here, none of my several /SR have the Medicine pool which is probable why I found it easier to drop it out of the build I offered than to try to work it in, particularly without extensively reworking power choices. It really boils down to personal preference and play style.
So here's another version including Aid Self. I also took Stimulant instead of Aid Another, which if it were me would depend on who and what ATs I most commonly teamed. It's so close to being soft capped I doubt one would notice a difference. And well it has Aid Self now for when one might.
Edit: Forgot to change back to a Doctored Wounds set on Aid Self when I swapped the Obliteration for Multistrike in Spin as the 5% bonus for recharge in Aid Self is no longer the 6th one.

Woot! If I don't run Tough, I can fight forever. Once I get my Miracle unique, I should be able to run Tough as well with no appreciable end loss. Fought a pylon for a few minutes to test end consumption, then I went in search of a RWZSC group! Killed some 53s, died to some lucky hits twice against a 54 group, and accomplished the RWZ Scrapper Challenge on the third try! WOOT! So, here's the final build I settled on. I'm still missing a lot of the LotGs, Aid Self still has generic IOs, and Health only has the Numina's unique and Heal and 3 generic heal IOs.

The easiest way to solve the endurance issue is to pick up Elude and Conserve Power... but I'm guessing you don't want those.

When I'm in a situation where I can be 2-shot, yes. Once I get the Miracle +recov (got my Regen Tissues +regen today, yay!), I'll just leave it on 24/7.
I like him a lot, but he's not to where I want him to be yet. I dish out pain pretty well, and rarely get hit (yays ez softcap). The problem is that I can bottom out my end very quickly on anything that's a melee Elite Boss or tougher (any AV, Rikti Pylons, etc).
