Go-live Known Issues list
[ QUOTE ]
Allies set to Melee with melee power sets will often try to attack enemies from outside their melee range. I'm not sure if this was an issue before i14 or not, but if it was, I've never noticed. With all the arcs that include allies with melee attacks, it's definitely a more apparent bug now.

That's what I was getting at with the 3rd bullet in the OP.
And for a while things were cold,
They were scared down in their holes
The forest that once was green
Was colored black by those killing machines

Hm. Strange, I must've missed the note. At least I know I'm not crazy, and the only one who's noticed that glitch.

Another big gotcha discovered recently:
If you edit a published mission from another computer from where it was originally published, and you didn't bring custom critter and custom group files to the new system, the edit will erase your custom critters from the mission. This is bad, m'kay? You won't necessarily notice your critters are gone until you playtest.

The forest that once was green
Was colored black by those killing machines
That's not so much a bug as it is a warning to make sure your other computer has the same info as the original.
Dec out.
No idea. Out in the middle of a big room. I didn't approach in a particularly careful manner even (as I've tried before when testing). I must have gotten lucky and just stopped at EXACTLY the right point or something. It was in one of those big three tiered office rooms with the staircases and the elevator on the third tier at the end (like the Green Fog office only normal). I was actually shocked to see it go off and not follow immediately by the "active" and being attacked.
Dec out.
