One "bug" I have noticed:
In Rikti War Zone, all quest giver NPC have .txt info type below their names on the compass now.
Also (not sure if this is working as intended or not) the Advanced Drones in the Rikti War Zone (mission instances anyway) when destroyed explode at least three or four times before going away....somewhat annoying. -

Don't use the 186.xx drivers. If you did, download the 185.xx drivers, uninstall the 186.xx, run DriverSweeper, and reboot. Cancel any Driver installations that come up, then install the 185.xx drivers instead.
The 186.xx drivers are causing problems for Windows 7 and Vista users. Another user downloaded the 186.xx drivers on XP and had no problems, so XP users should be fine.
The problem that mulletman has is different, especially since he is using the newest drivers for his video card already.
I'd recommend trying to run the game in Safe Mode to see if that will let you into the game. If it does, go into the Menu > Options and on the Video tab set it to run in Windowed mode.
If you can't get there using Safe Mode, download Tweak CoH and use it to set the graphics settings before loading the game. You should be able to set it for Windowed mode from there.

Repectfully, one problem with the above....CoH/CoV is the only problem that these drivers are causing problems with under Vista. Haxi2 has about a dozen other games (including other MMO's) which are not having ANY problems with the new Nvidia drivers. This is a CoH problem and not a Vista an Nvidia problem.
Frankly, I would not have advised anyone to download the new drivers since they are so new. This usually happens when new drivers are released. I doubt NCsoft has had a chance to see what the problems are that they have to address yet. Hopefully, they will release an update soon that will fix this issue. Haxi2 will not be rolling back his drivers for only one piece of software that does not play nice. If NCsoft refuses to fix the issues from their end then he will be cancelling his subscription as will I. -
I posted this in the "other" Vista problems thread but Vista SP2 does not fix this issue. It can not fix this issue as it is a problem with Open GL 3.0 and the CoH/CoV client. NCsoft needs to fix the problem and stop saying it is due to Vista.
I would also like to point out that I have had 3 to 4 days of no problems before this issue comes up again. Then, it may occur several times within an hour or once per day or whatever. This is SP2 is not the "fix" as advertised (so speak). -
This problem is not simply a Vista issue. Installing SP2 and/or reinstalling drivers should not be the way you have to fix this problem. The problem is related to Open GL 3.0 and changes which have not yet been addressed by CoH/CoV. Simply speaking, the game has not been kept up to date for the operating systems available. While NCsoft has invested in a Mac Client, they have not updated the Windows clinent to work properly under Vista. I have decided that since NCsoft is not doing anything to fix this problem, I will be cancelling my account. I should note that other older MMO's (older then CoH/CoV) have addressed these issues and crashes/lockups of this nature do not occur. So, as of the end of July I will be out of this MMO.

Funny, 5 computers in my house all run vista save the windows home server. All on vista 64 bit ultimate edition with service pack 2, full version from technet not the beta version.
I dont have any crash issues. 3 of my computers are laptops with ati and nvidia integrated graphics and no crashes.
My bet, you have a hardware issue. Possibly a cheap video card or a chipset fan not working.

I just wanted to add that I had been crashing sometimes several times within a normal play session prior to applying the latest Nvidia graphics drivers (182.50) for Vista 64. Now I occasionally still crash but it seems the frequency has gone down.
In regards to the Microsoft hotfix as suggested, I have not applied it because of this (from the support.microsoft.com article):
"Windows Vista hotfix information
A supported hotfix is available from Microsoft. However, this hotfix is intended to correct only the problem that is described in this article. Apply this hotfix only to systems that are experiencing this specific problem. This hotfix might receive additional testing. Therefore, if you are not severely affected by this problem, we recommend that you wait for the next software update that contains this hotfix."
Note that it suggests that you NOT install the the fix UNLESS you are being serverly effected. While I wish CoH/CoV not to crash at all, I am not effected in other software apps so I will not apply the hotfix. -

Hot fix hasn't addressed it. Yesterday I played most of the day and then crashed twice within an hour. Today, I play about 4 hours no problem and then crashed about four times within a two hour period. I am a Vista 64 user and game is still crashing (video driver has stopped working). Same is happening for two others that I know.
We just got into CoH/CoV recently as a group. We are becoming a little disheartened by this issue. I will not continue my subscription if this is not fixed realitively quickly. I play other MMO's on this machine and never crash....and one of those is older then CoH.
