I do not see your CoH Helper log, so I am going to make a few guesses here.
Correct me if I am mistaken;
You are using an AMD/ATI graphics card, and are not using the 11.9 drivers?
If the question is true on both counts, try installing the 11.9 drivers and see if that helps.
You might need to cleanly install the drivers to get them to "stick" (to use MMO terminology). Driver Sweeper is the go-to program on these forums since it is free and well tested.

Every single time I played City of Heroes, after a while of playing, would simply freeze and then I'd have to bring task manager to up to shut it down and restart the game to keep playing.
This has become tedious and now I want a solution.
I am not restarting the game every 20 minutes. Ridiculous.
