Completely black screen when loading the client
Delete your game preferences (Users > yourname > Library > Preferences > City of Heroes) and try again.

I re-read your description - sorry I thought it was something common, but it's another issue entirely.
First thing, what kind of Mac are you using?
Second, just to see if it works: Start CoH, click Next but then click the safe mode checkbox before clicking Agree. Then set up your graphics settings.

I have this problem from time to time, it is frustrating.
I find if I force quit the application from the apple menu and restart the client again, it will usually work as normal. But the black screen is a frequent occurrence!!!
I've just installed Coh under OSX 10.6.4. I pulled the client from client.coh.com yesterday.
When I go to launch, the screen goes black. It still is getting signal but it's totally black. I can hear the music playing properly. One time I could see the game dimly, enough to login, try to change some options (crank the gamma up), and log out, but not since.
I can't Cmd-Tab, Cmd-Alt-Esc, or get any other keyboard command to work.
I was able to ssh in from another machine. I had 'wineserver' running with no switches, as well as this for the wine command:
/Applications/City of Heroes.app/Contents/MacOS/cider_noui -cmdline c:\coh\CityOfHeroes.exe -project "Coh" C:\coh\cityofheroes.exe
I have not edited any switches, just running out of box.
If I kill the process, I am returned to the Blue desktop, but Finder doesn't reload, and I have to power cycle the box.
I have attempted the empty cache/cache flush/delete prefs, and that doesn't have any effect. I have also deleted and reinstalled the client, also to no effect.
