Floor trim breaks in doorways and near objects

No on both questions unfortunately.
I usually go for the no trim look by putting small wall items(single cable or small half round vent) in places where I don't have big things taking away the trim. If you sink them most of the way into the floor or roof you can usually hide them pretty well.

I am just starting out in base designing with what I have to work with, and I have noticed that the disappearing floor trim is really starting to look tacky.
Is there any way to get around this besides moving items away from the wall a bit, or get the trim IN doorways?
