I just petitioned my unawarded Vet badge last night as well. Finally got an email about it again (haven't been getting support emails for the last 6+ months).
I'll give them until tomorrow morning and then update the ticket.
And Dumple, my vet rewards look very similar to that, showing that I earned the 60 and 66 month badge on the same date. 
Main Hero: Chad Gulzow-Man (Victory) 50, 1396 Badges
Main Villain: Evil Gulzow-Man (Victory) 50, 1193 Badges
Mission Architect arcs: Doctor Brainstorm's An Experiment Gone Awry, Arc ID 2093

I have asked permission from the GM that handled my ticket to post their response. No word yet, but the gist is basically that they are working to implement a new behind-the-scenes Vet Reward system that won't get corrupted like this anymore. I'll post the response here if I get the word.

Hello, In order to correct the issue you faced with your rewards, it was necessary to also reset your past rewards so that everything came through. This is the reason that a lot of your reward dates appear to be off. We are aware of the display issues caused by this issue and the Development Team is currently working on a more permanent resolution for this issue. Once a fix has been completed and tested, it will be made available in a future patch to the live servers. Please keep posted to the City of Heroes/City of Villains message boards and website at http://www.cityofheroes.com or http://www.cityofvillains.com for any updates to when a fix will be implemented. I just got a double-escalate email followed by a quick reply that "an error" had indeed occured and that my account has been fixed. It will have to wait until I get home to verify but Huzzah!
less that 1 week to 698 (last day job +accolade) blue side
and less than 24 days to 620 (last 2 day jobs +accolades) red side
But speaking of red side: Notice that Titan Network doesnt recognize "Lost Savior" for those of use crafty enough to get it red side. Badge-Hunter does.
