Team Listing Innacurate

Since the last patch I've seen problems with the team listing:

My character name listed twice, in separate zones (one of which I hadn't even gone to in weeks)

Other character names blank when in another zone

Together we entered a city of strangers, we made it a city of friends, and we leave it a City of Heroes. - Sweet_Sarah
