I'm not an IO guru, so I'll leave slotting discussion to the experts. However, I've got a couple comments on your power choices, based on the journey of my DB/WP to 50, including a few post-50 respecs.
First off, I'll second what LVconvert said about focused accuracy - if you're running both stamina and QR, Conserve Power is overkill. DB needs the extra accuracy more than most primaries, if only for the sake of stringing combos together, and with both endurance recovery powers running, another toggle won't hurt your blue bar too much.
Speaking of combos, I would recommend working OTC and the Sweep combo into your attack chain, at least against larger groups. The knockdown from sweep is a valuable form of mitigation. Especially if you aren't going to take the medicine pool, it gives you a chance to breathe and recover some health while the mobs are on their backs. It does bring down your DPS a little, but it keeps you safer. My preferred attack chain is:
BF > Attack Vitals > sweep, although I'll skip sweep if I'm fighting something with enough knockback resistance.
Lastly, I don't really think that DB/WP has much need for Hasten. The only WP power with a long recharge is Strength of Will, which like you I barely ever use. And the DB attacks all recharge quickly enough that you only need a little bit of global recharge from IOs to work through your attack chain pretty easily, even if you leave out the sweep combo.
