I'm not aware of any cooldown period of procs and yes, each hench would have the chance to hit (per each target hit). For instance, I run Soulbound Allegiance: Chance for Buildup on my Assault Bot and I frequently see it double-stack.
That said, have a read of my guide on henchmen and procs, as there are some things to be aware of when slotting them (namely, what powers can make use of a slotted invention set enhancement).
There was also a good overview posted by Biowraith recently, here if you want a short answer. -

They are two variables in the maths used to judge if you hit something you use a power against.

Essentially for all the layman needs to know (like myself), ToHit bonuses are more powerful (unit for unit) than Accuracy bonuses. -

Confirmed resolved for Protector Bots/Assault Bot - completed "Defeat Rikti Leader" with fully upgraded henchmen.

I'd be curious if any of the testing we've done has contributed to helping the devs fixing the issue or whether we should have just left them to it.
To be fair, I'm going to feel a little disappointed if the 30 mins or so I took out of playing to narrow down the problem wasn't productive for them (and would disincline me to troubleshoot on Live in future). -

My theory so far has seemed fairly solid (and is cast-iron for my own experience - I can run with all my level shifted bots so long as only Drones get the L32 upgrade and no other type) -
So, under the forum assumption that the issue is to do with pets in some capacity, I started some testing in this vein. The mission used was "Defeat the Rikti Leader before he attacks" from Borea in RWZ.
When the mapserver occurred, it would do so on engaging the first spawn. Difficulty was set to +0/x1 (as I was testing varying amounts of pets).
Henchmen were summoned/upgraded on initial entry to the mission. The slotted Rare was [Nerve Partial Radial Revamp].
Fully Upgraded
Battle Drones + Protector Bots + Assault Bot = Mapserver
Battle Drones + Protector Bots = Mapserver
Battle Drones = No Mapserver
Assault Bot = Mapserver
Unupgraded
Battle Drones + Protector Bots + Assault Bot = No Mapserver
Battle Drones + Protector Bots = No Mapserver
Battle Drones = No Mapserver
Assault Bot = No Mapserver
So, for Masterminds the initial problem appears to stem from the interaction of a level shift with the henchmen upgrade powers on Protector Bots and Assault Bots. The unupgraded test runs could be reliably crashed by applying upgrades to the bots and then engaging in a fight with them. Except for Battle Drones.
So, testing down this line of thought, I was able to fully upgrade the Battle Drones then partially upgrade Protector Bots and the mapserver was stable. It only crashed once I applied the L32 upgrade (after a couple of fights) and then shortly after they summoned Seeker Drones (a power granted by the L32 Upgrade). So, I then tried...
Fully Upgraded Battle Drones + L6 Upgraded Protector Bots + L6 Upgraded Assault Bot = No Mapserver
I was able to force the mapserver as soon as I applied the L32 upgrade, and again the mapserver occurred shortly after the L32 Bot powers were used.
I would therefore conclude, initially, that the cause of the mapserver issue is summoned entities by a summoned entity that is level shifted - both Assault Bots (in Missile Patches) and Protector Bots (Seeker Drones) cause a summoned entity with the L32 Upgrade, whereas the Battle Drones do not.
Does this tally with anyone elses experience? -
For me, I'd like to see the following options on apppearance...
- Match Primary - a themed pet matching Mastermind or Controller pets in appearance
- Match Alignment - same as the Backup Radio craftable temp
- Match Character - a doppelganger of your own self (not necessarily with your powers)
- Custom Character - complete custom appearance
Myself I'd prefer something like a HVAS or WarWorks robot- so wouldn't want a custom character, rather something to match my primary. Hence my wishlist- caters for most tastes I think! -

I think everyone is getting alpha boosts since it's the first day, and crashes are happening a lot for un-related reasons, and it's unfortunate that some people are losing their rewards.
I appreciate that perhaps not everyone is affected or that it isn't every map that will have a mapserver crash (hence why I've been posting mission titles) but I don't think this can be blown off as a non-issue. -
Unfortunately, this wasn't supplied and now we have a fairly major /bug out in the wild. Please, please allow effective testing in future - especially on new systems like Level Shifts!

I'd recommend not using a Rare Alpha slot at all inside of an instanced mission while Paragon Studios look into the issue. -
Tested, yet again using my [Nerve Partial Radial Revamp] in a Villain Tip mission (Attack Nemesis Base). Ran to first spawn (after upgrading henchmen) and...
So, I log back in and give it another go. Run straight to the first spawn (once again after summoning henchmen) and...
So, after another attempt (not screenied) I switch down to a freshly crafted [Nerve Radial Boost] and what do you know...
... all is well, spawn defeated and I cleared the mission no problem. So, despite the now official announcement it is NOT limited to the Weekly Strike Target. -

I've encountered the same issue in a RWZ Borea mission ("Destroy Rikti equipment").
With my new [Nerve Partial Radial Revamp] I consistently mapserver'd on the first spawn after four attempts. As soon as I switched to a [Musculature Core Boost] I was able to continue past the first spawn. -
Just finished a LRSF and I can confirm MM's are A-OK. Screenies to follow...
Edit: Okay, here's the proof. Your bots will actually get an Auto power for the rare you slot...

Back on topic for, you know, actual testing of the given system (rather than suggestions for changes) has the US Test Server had a WST announced yet?

Devs,
In order to actually assist testing on the latest patch, we need the following on the EU test server...
- Fix the Character Transfer system
- Issue a Weekly Strike Target
- Change the /gmotd to reflect the new target
- (Suggested) Offer a test-server only 200 shard-dump (as we know shard salvage works, we only need to test the new crafting, breakdown, etc. so we need some way to avoid shard grinding unless we're expected to game exclusively on Test for a bit!)

Alas, the Weekly Strike Target on the Test Server has yet to be announced. Once it is I'll try and scrape a team together on the EU Test Server to have a look at this, but some MM's on the US Test Server may be able to get this done quicker (since the EU character copy tool is broken)
