This is also from the ticket I have open for the black screen issue. It may be related to the problem we're getting (or may not). However, please let me know if you've experienced the same thing:

Here's how to replicate it:
1. launch CoH, login, select a server, select a character, then click Enter City of Heroes
2. Switch from fullscreen to windowed mode
3. Switch from windowed to fullscreen mode
Here are some additional findings:
1. Prior to clicking the Enter City of Heroes button after selecting a character, if you switch between fullscreen and windowed mode, screens are rendered normally. This is true for the login, server selection and character selection screens.
2. After you select a character and click on the Enter City of Heroes button, if you logout to the character screen, server select screen, or login screen, then attempt to switch between fullscreen and windowed mode, it will result in a black screen in fullscreen mode. Additionally, screens will render properly in windowed mode, but not fullscreen.
3. It appears that the only way to have the client work properly after switching between fullscreen and windowed mode, while logged in on a character, is to restart the client.
4. In the character selection screen, you can select any number of characters (in my test, I selected each character I have on Freedom twice, and switched between fullscreen and windowed mode each time) without getting the black screen. It's only after clicking the Enter City of Heroes button that the problem starts. -
At the suggestion of the support folks at NCSoft, I installed the test client, and modified it to connect to the live servers. Thus far it resolves the black screen issue.
Here's how to modify it. I'd be curious to see if this works for others as well.
f you are currently connecting to the test servers, please try the following to connect to the live servers:
1. Open your Hard drive.
2. Open Applications.
3. Control-Click on the City of Heroes.app and choose "Show package contents".
4. Open Contents.
Now, I would like you to modify the info.plist file that is located in the Contents folder and remove the following lines:
<key>CiderGameOptions</key> <string>-- cohupdater.exe -test</string>
Also, please open a ticket to let the devs know if this works or not. It may get us a fix much faster. -
In my case, I don't even make it to the initial splash screen, loaded before the login screen comes up.
I've received pretty timely replies from support; they're looking into the issue, but it just may take some time. My guess is that the character slot issue is a higher priority in the grand scheme of things (not that it makes me happy, but I understand).
Heck, both WoW and Eve Online have had their share of hiccups with major content releases... it seems par for the course for any complex MMO.
I also have to commend the community here. Were the WoW or Eve Online, there would have already been cries for blood. Here, people are reasonable and mature (did I just open a can of worms?). -
I'm having the same issue. I opened a ticket with support.
My fear is that it may be related to this thread, regarding driver issues on for PC video cards.

They at least have the recourse of downloading & installing the latest drivers. -
Here's my configuration:
- Mac Pro 1,1 w/ dual 2-core 2.66Ghz Xeon procs (4MB L2 cache)
- 10GB RAM DDR2 667Mhz RAM
- ATI Radeon 5770 1GB
- Razer Naga Mouse using driver 2.0
- Razer n52te using driver 1.0.2
- OS X 10.6.6
- dual 20in apple cinema displays (1680x1050)
- 3 1TB drives in RAID 0 array (yes, I know this is insane)
Like some others, I too have the issue where CoH will quit silently. In rare cases I'll even get an assertion error.
The only mode I can reliably run in is performance mode; if I run in ultra mode, I'll get about 15 minutes of game play before it quits.
As an interesting aside, if I switch to windowed mode, then back to full screen, all I get is a white screen until I restart the client.
Thus far I've tried the following to resolve the problem:
1. Reset my prefs by renaming ~/Library/Preferences/City\ of\ Heroes
2. Ran in Safe Mode
3. Restarted
4. Ran fix disk permissions
5. Ran fsck -fw from single user mode
6. Reinstalled CoH from the lastest installer
What I still need to try:
1. Run the 10.6.7 update
2. Disable spaces
