Great discussion thread. I'll have to hop back onto my Son/Son as soon as I replace more of the sound FX associated with it.
It's very headache inducing at the moment. -

I cringe whenever i'm in a pug that starts requesting spesific powers.
We need a stone tank... when any tank or more buff/debuff will do.

I have little tolarince for people trying to dictate how others play or what they should play. I always suggest to teamleaders, hey, that TA over there can debuff well enough, grab him, or, that fire tank is fine... ect. Most listen, some don't. *shrug* -
I wonder if the dialog box for the second one was hidden behind the first one. I almost deleted a global friend once because the 'OK' button for another dialog was on top of the 'Delete' button. It seemed that as far as mouse-clicks were concerned, both windows were active.
Was your machine lagging at all? If so, it may explain why the rt-click dialog for the second recipe might have been up. Initially, they all pop up in the same location.

I've actually had the WW porter dialog box disappearing on me instantly. Was standing in one of the Portal Corp buildings, activated the WW porter, it's dialog box opened and promptly diapered before I could even move my mouse. -

Toggle-heavy squishies or ATs that lacked mez protection but relied on toggles for survivability (see Khelds) got the short end of the stick, especially in a game where micro-mezzes (Crey and KoA are the most suspect groups here) in the form of sleeps or very short-duration holds caused the huge annoyance of having to retoggle everything.
Given the current PvE mez and toggle rules, people needn't complain about micro-mezzes anymore, and standard mezzes are just a small hurdle that's easily overcome with a Break Free or two. If you don't regularly carry them when you know you're going to be facing mezzing enemies, then you probably deserve whatever debt you end up getting. Inspiration management's not just a PvP skill.
