The First Ward contact Nadia, gives a mission titled Investigate The Slaver Den. After completing it, you automatically get a second mission to find Anna in the Shepard's Haven.
The mission to find Anna will persist between play sessions (logging off and on) but the Door ID becomes invalid once the character logs off.
This results in being unable to enter the mission once the character logs back on. Mission Teleporter powers (temp & perm) also fail and no mission marker is placed on the world map or minimap.
The only solution I've found thus far is to abandon the Anna mission and re-do the Slaver Den mission.
