It usually means there's something between your click-target and the camera view. Usually it's something as stupid as a hanging cobweb, or the targeting box of an enemy or ally just out of camera view.

Yes it's something behind you. I get that a lot on my masterminds when my pets are on follow, I have to stand back before I can click on a door or a glowie.
I've noticed that sometimes even though a defeated enemy falls down, their (invisible) targeting box remains upright and gets in the way of me trying to select someone behind it.
Teams are the number one killer of soloists.

Sometimes my mouse pointer turns red and I can't target with the mouse. Tab targetting still works.
What exactly is going on when the mouse pointer turns red? Sometimes it happens with targets standing behind me, so does the mouse pointer turning red have to do with not being able to see that target?
With the servers down for five hours, figured I had time to finally ask.
