There's probably dudes hanging around somewhere that you just haven't looked yet. That's in an Arachnos lab map, isn't it? Those things suck for hiding critters. Not to mention a lot of them have geometry holes.
You may be stuck either dropping the mission or petitioning for GM assistance.

So trying to finish the Steal book from Ghost Widow mission, and I have slaughtered everything (including the reinforced doors I can reach), and it still says kill all operatives... Is this normally a bugged mission or am I missing something.
I have gone around hitting TAB looking for a person I could have possibly missed to kill... there is no one.
