I've clicked on all the 'known' plaques, including one that looks like it should be the 'Just say no' one, but no history badges.

Is there an NPC somewhere you must talk to, or the like?

If anyone's finished a history badge, they haven't said anything. But then, it doesn't seem like any of them will have just a single plaque to find - here's an example, seems to be about the life of Atlas.

I've also found three plaques relating to the Citizen's Crime Fighting Act, three involving Hero Corps, two about a Lost boss named Ishmael, and four that look to be about the Family and Superadine.

Others having problems - it's not the destroyed billboard at all, there is an intact sign. This screenshot shows what you're looking for.

Same here. Is there an intact Crey sign around there somewhere that I'm not seeing?

Earned these two from going afk for a while with Unyielding Stance on.

Hero Corps Insider(badge) you get this one when you get near the Hero corps dude in Atlas park near City Hall( sorry not ingame but i think ya know which one it is , he gives those Vahz lowbie missions)

Also, the Hero Corps woman outside the hospital in Steel Canyon works. Forget her name. (At least I got some hero corp badge for getting near her, and pretty sure it was the same one).

It's not the same one, she gives the Hero Corps Recruit badge.
