Teamed: Happens when teamed and when not teamed.
When do you crash: At points where it's required something loads. Missions and other maps it always happens, I'll enter but the loading screen will stay up with no progress being made on the bar at the bottom of the screen. After several minutes the screen will come back to where my character had just entered and the message "No mapServer connection. Try again." will come up in a text box. I've also had difficulty if this happens when going into a zone in reloading that character. The character select screen will freeze for several mintues before the same error message will come up and I'll be kicked back to the login screen.
